When you submit our interest form, we collect:

• 👶 Child's age group (not the child's name or contact info)
We do NOT collect medical information on this form.
We do NOT collect personal info directly from children.
We use your information to:
• 📞 Contact you about the program
• 🗓️ Schedule your introductory call
• 📬 Share helpful program information
You may opt out of communications at any time.
This form is for parents and legal guardians only.
• 🚫 Do not enter your child's email or phone number
• 🚫 We do not knowingly collect information from children under 13
If you join the program, we will collect health information later during secure onboarding.
At that point:
• 🔐 Health data will be protected under HIPAA
• 🧾 Additional consent forms will be provided
This interest form is not used to collect medical data.
We take steps to keep your information safe:

• 👥 Limited staff access

We use minimal analytics tools to understand how visitors use our website and to measure the effectiveness of our advertising.
